Acknowledgment of Risk:
I, the undersigned parent or legal guardian, understand that participation in youth soccer and futsal activities carries inherent risks, including but not limited to physical injury, illness, property damage, or, in rare cases, serious harm or death. I affirm that my child is in good health and physically capable of participating in all activities offered by DC Flash Academy.
I knowingly and voluntarily assume all such risks on behalf of my child, including those arising from the negligence of DC Flash Academy or its staff, volunteers, coaches, or affiliates.
Assumption of Risk – Communicable Diseases:
I acknowledge the inherent risk of exposure to communicable diseases, including but not limited to COVID-19 or other viruses, illnesses, or mutations thereof. Despite preventive measures, I understand that it is not possible to eliminate the risk entirely, and I voluntarily assume all such risks on behalf of my child.
Medical Authorization:
In the event of a medical emergency where I cannot be reached, I authorize DC Flash Academy staff to seek medical attention for my child. I understand that I am financially responsible for all medical expenses incurred as a result of such treatment.
Release of Liability:
In consideration of my child’s participation, I hereby release and hold harmless DC Flash Academy, its directors, officers, staff, volunteers, coaches, agents, and affiliated entities from any and all claims, liabilities, demands, causes of action, or damages related to or arising from participation in any program or activity, whether caused by negligence or otherwise.
Media Release:
I grant DC Flash Academy the right to use photographs, videos, or audio recordings of my child, taken during participation in any academy activities, for promotional purposes including but not limited to social media, website, flyers, or any other lawful marketing or communication channels. No compensation will be provided.
Binding Effect:
This Waiver and Release of Liability is binding upon myself, the participant, and our family, estate, heirs, executors, administrators, and assigns. I further agree that this waiver is intended to be as broad and inclusive as permitted by the laws of the State of California (or applicable jurisdiction), and if any portion of this agreement is held to be invalid or unenforceable, the remaining terms shall remain in full force and effect.
